8 Norman, OK Norman, OK Part-time Full-time Part-time Full-time Up to $38 an hour Up to $38 an hour 5 days ago 5 days ago 5 days ago Halo Hospice is seeking an RN Admissions Coordinator whose primary responsibility is to educate on the benefit of hospice services and on hospice philosophy to patients and families in a manner that results in the admission of appropriate patients.
We are looking for a compassionate and caring RN that wants to make a difference in the lives of the patients and families we serve.
Responsibilities:
Evaluates patients for hospice appropriateness, to include thorough medical records review and other current resources Provides hospice 101 as needed, assists patient/family signing legal paperwork for hospice admission, completes admission process and nursing assessment Educates patients and caregivers regarding the hospice program, philosophy of care, obtains informed consent, election of benefit, and reviews patient rights and responsibilities and other required federal/state documents.
Performs initial assessment of physical, environmental, spiritual, bereavement and psychosocial factors to determine hospice patient and family needs upon admission and establishes the Plan of Care.
Coordinates the establishment of the initial plan of care with the attending physician, the Medical Director, and the Interdisciplinary Team.
Coordinates the initial Plan of Care and maintains continuity of patient care through excellent communication with Hospice team members.
Coordinates for the provision of DME, supplies and medications identified in the initial plan of care.
Manages post admission tuck-in program Supports 11th hour services as needed Qualified candidates include being a graduate of an accredited Associate or Baccalaureate School of Nursing; Current Oklahoma state license as a Registered Nurse; Current Oklahoma Driver's License; Hospice experience required; Halo Hospice has been serving central and south central Oklahoma for over 17 years!
